Access the SayPro Survey Tool from your dashboard and choose the template for Community Meeting Feedback. Customize the form to include:

  • Event Details (auto-filled from the event scheduler)
  • Participant Details (optional: name, ward, affiliation)
  • Core Feedback Questions (suggested fields):
    1. Was the meeting well organized? (Yes/No/Neutral)
    2. Did you feel heard and included in discussions? (Scale 1–5)
    3. Were the topics relevant and clearly presented? (Scale 1–5)
    4. What was the most valuable part of the meeting? (Open-ended)
    5. What can we improve for next time? (Open-ended)
